Access Request

This form is used to request access to SISS systems, which includes DukeHub, DSV, Perceptive Content, and Slate, and should be completed by a supervisor or manager.

Note: This form is required for additions, deletions and modifications to user security.

The New Program Form is provided to all University Departments that expect to add a new academic program or degree offering in the near future.

Note: New programs should go through the normal processes of review and approval with the Academic Council.
